Please select the appropriate Tax Organizer from the list below. When prompted to Open or Save, select Save and save the file to your Desktop.

When you have filled out the form and are ready to send it back to us, in Word click on File > Save As and add the first four letters of your last name and last four digits of your Social Security Number to the file name.
